Can I replace a damaged, lost, stolen, inactive or unreadable Chick-fil-A Gift Card?

Yes, damaged, lost, stolen or inactive Chick-fil-A Gift Cards are subject to replacement upon request. Please complete the Gift Card Replacement form here. Completed forms can be mailed or emailed to the addresses listed at the top of the form. For lost, stolen or inactive gift cards, you will need the original sales receipt and/or activation receipt to determine if your gift card can be replaced.
